Reasons For Breast Implant Removal
Breast Implant Removal is an uncommon procedure. It is estimated that around 1% of all patients with breast implants will have them removed during their lifetime. This includes reconstructive and cosmetic patients. Reasons for breast implant removal include infection and exposure of the breast implants. These are rare, but most commonly seen in patients who have had breast reconstruction after a mastectomy for breast cancer. The risks are further increased by radiation treatments, which progressively injure the blood vessels supplying oxygen and nutrients to the breast tissue.
Complications like infections and exposure of the implants are less likely for cosmetic patients with healthy breasts, but you can increase these risks by choosing breast implants that are too large for your frame. Extreme breast implants overstretch the breast skin and compromise breast perfusion. The injury is similar to that caused by radiation and mastectomy surgery.
Implants that are too large are another reason for Breast Implant Removal. This can happen early, because the implants selected were just too big. Or this can happen later, due to weight gain or hormonal changes which can increase the volume of the natural breast tissue. For some patients, they are just done with breast implants and want them removed.

What To Look For In Breast Implant Removal Before And After Photos
What does Breast Implant Removal surgery do? Below are selected Breast Implant Removal Before and After Photos that demonstrate actual results. Several alternative techniques are used. Each procedure includes removal of the Breast Implants without replacement. Some include partial or full capsulectomies, en-bloc resection and lifts. Cosmetic results after the removal of smaller breast implants, especially when associated with larger volumes of natural breast tissue, tend to be better. Since the implants make up a smaller percentage of the breast volume, the loss of breast support is less dramatic. Cosmetic results after the removal of larger breast implants, especially when there is thin skin coverage, are more likely to be disfiguring.
Sometimes a Breast Lift is recommended to compensate for the anticipated sagging. Breast Lifts may be performed in conjunction with the Breast Implant Removal, or a delayed lift may be recommended. Breast Implant Removal Before & After Pictures: Case 1
Breast Implant Removal Before and After Photos: The primary goal of Breast Implant Removal is to decrease the size of the breasts. While Breast Augmentation enlarges and supports, Breast Implant Removal does the exact opposite. It reduces breast volume and allows the skin to sag. In cases with small implants and good skin tone, the results are usually aesthetically pleasing smaller breasts.
Before: This patient was unhappy with the size of her breasts and wanted to reduce them. She is thin with a small frame and felt her Breast Implants were out of proportion from early after augmentation. Prior to Breast Implant Removal, her nipples are sitting well above the inframammary folds (IMFs). The areolae, like the breast skin, are slightly stretched, which both enlarges and lightens them. Additionally, the majority of her breast volume is supplied by her breast implants, which can be a problem after the implants are removed. Fortunately, she is young and has excellent skin tone.
After: The breasts are significantly smaller, but thanks to her excellent skin tone, there is no sagging postoperatively. Her nipples remain well above the IMF. Also, her areolae are slightly smaller and darker after the stretch from the Breast Implants was eliminated. Incisions around the areolae from her original breast augmentation were used for her Breast Implant Removal.
Breast Implant Removal Before & After Pictures: Case 2
Breast Implant Removal Before and After Photos: This case is similar to the first; however, the breast implants are larger and the skin is more lax.
Before: In this case, the breast implants are supporting the skin and breast tissue. Larger implants provide more support. The nipples are at the front of the breasts, which helps with the aesthetics after Breast Implant Removal.
After: The support provided by her breast implants is lost after Breast Implant Removal. The upper pole of the breast completely deflates, and the projection of the breast is greatly reduced. As the breast deflates, the skin on the front of the breasts drops. The nipples, still at the front most portion of the breast, also descend. These breast implants were removed via the IMF incision, the most commonly used incision for Breast Implant Removal.

Breast Implant Removal Before and After Photos: This is another example of a medium sized breast implant removal in a patient with relatively good skin tone and no residual sagging.
For Breast Implant Removal, skin tone is the most important predictor of a good result. However, when smaller breast implants are removed, the skin tone becomes relatively less important.
Before: This patient was unhappy with her breast augmentation. She felt it was too much and was very self conscious with her appearance. Prior to her Breast Implant Removal, her breasts were firm and perky, with her nipples just above the most anterior aspect of her breasts.
After: Breast Implant Removal was performed via a periareolar incision. She had saline filled breast implants. The saline was drained before the implant was removed. The deflated implant shell was then removed through this small incision. Due to her good skin tone, the breast skin collapses against the chest wall instead of drooping down. Her nipples remain above the most anterior projection of her breasts.
Breast Implant Removal With Breast Lift

Breast Implant Removal With Breast Lift Before and After Photos: Breast Implant Removal was combined with a Breast Lift in this case. Simple Breast Implant Removal would have left the nipples high and completely above the breast mound. In this case, the incision used for the Breast Lift is longer than the incision normally used for simple breast implant removal; however, the longer incision allows for an en-bloc, complete resection of the breast implant still within its capsule.
Before: The breast implants have bottomed out leaving the nipples high, and the breast mounds low on the chest. The skin was thin, especially over the upper pole of the breast implant. Straight Breast Implant Removal would have left no tissue beneath the nipples and pan-caked breasts.
After: Breast Implant Removal with a Breast Lift was used to lift the breast tissue in the lower pole of the breasts and support the nipples. In this case, a single incision was made along the IMF. It is longer than the standard breast implant removal incision, but allowed for en-bloc, complete resection of the breast implants in their capsules. This is great for removing old implants and containing the leaking silicone gel. All implants manufactured before 1994 can, over the years, bleed silicone oil through the intact shell. Moreover, implants made before 1985 break more than half the time and release their oil-like contents into the capsule. Newer "Gummy Bear Breast Implants" are filled with a cohesive gel which does not migrate like the previous generations of breast implants, even in cases of implant rupture.
Breast Lifts may be combined with Breast Implant Removal, but sometimes it is recommended to perform the lift later. Doing the Breast Lift as a separate operation gives the breasts time to recover and the skin more time to shrink.
Breast Implant Removal Before & After Pictures: Case 5
Breast Implant Removal Before and After Photos: This Breast Implant Removal patient has ptosis (sagging) before her procedure. A Breast Lift was discussed, but she wanted her breast implants removed first. The plan was to reevalute the breast ptosis after she was healed. The sagging is increased after surgery, but it didn't bother her. The upside is the scars are minimal, and she retains the option for a lift in the future, should her mind change.
Before: This patient complained of loose, saggy, heavy breasts, similar to women without breast implants who desire Breast Reduction. She wanted smaller breasts but did not want breast lift scars, as she was not concerned with the amount of ptosis present before surgery.
After: Breast Implant Removal was performed to reduce the size and weight of her breasts. While the nipple now appears lower on the breast, it is not lower on the chest. The breast has collapsed and both the upper and lower poles are smaller. She is left with both ptosis (a low nipple) and pseudoptosis (most of the breast tissue below the inframammary fold), but she looks natural. She was happy with the resultant size and shape, so a delayed Breast Lift was not performed.
Incisions Used For Breast Implant Removal
Breast Implants are usually removed via an incision along the fold under the breasts also known as the IMF (inframammary fold). If you already have an IMF scar from Breast Augmentation, it can be used for Breast Implant Removal. The periareolar scar, the scar placed around the edge of the pigmented skin around the nipple, can also be used; however, there is an increased risk of dimpling and tethering of the scar in this location. This means you could end up with a permanent indentation of the scar, which may pull in with muscle flexion. Axillary and umbilical scars, from the transaxillary and transumbilical (TUBA) approaches, are usually not suitable for breast implant removal. Radical, en bloc resection of the breast implants while they are still in the capsule requires a larger incision, which is placed along the IMF. En bloc resection is helpful in cases where silicone breast implants have ruptured, especially those manufactured before 1985. At the time of surgery, if the capsule is found to be thin or tightly adherent, en bloc resection may not be possible, and alternative methods are used to minimize leaving silicone gel behind.
Breast Implant Removal can be performed for both silicone and saline breast implants. The incision is usually shorter for Saline Filled Breast Implants as described above in case 3.
