Reasons For Breast Implant Revision

Breast Implant Revision is a common procedure. It is estimated that around 25% of all patients with breast implants will have them revised during the first 10 years after their initial cosmetic Breast Augmentation. The risk is doubled for reconstructive patients. Reasons for Breast Implant Revision Surgery range from completely voluntary to emergency surgery, though Plastic Surgery emergencies are rare. The most common voluntary Breast Augmentation Revision Surgery is to increase the size of otherwise satisfactory breast implants. This may account for one-third of all revisions. Other common reasons include: Implant Malposition, Capsular Contracture and Breast Implant Rupture. Infections around breast implants are rare but may require urgent revision surgery. What To Look For In Breast Implant Revision Before And After Photos

What can Breast Implant Revision do for you? Below are selected Breast Implant Revision Before and After Photos that demonstrate actual results. Many techniques are used, and often they are combined to achieve the best results. Many revisions utilize the original breast implants, while others may include removal, replacement or changes in the size of the Breast Implants.

Differences handling the pocket that the implants sit in, the capsule, are also demonstrated. Some may include capsulotomy, partial or full capsulectomy, capsulorrhaphy and/or neopocket formation:

  • Capsulotomy - Scoring or dividing the scar capsule around the breast implant to enlarge the pocket.
  • Capsulectomy - Partially or entirely removing the capsule around the breast implant
  • Capsulorrhaphy - Tightening the capsule or adding a sling of tissue or prosthetic mesh to tighten the capsule and support the breast implant.
  • Neopocket Formation - elevating the capsule and forming a new pocket, usually behind it.

Cosmetic results after Breast Augmentation Revision often depend on the health, vascularity and elasticity of the skin, breast tissue and muscles of the chest. Since larger implants have more negative effects on these variables, revision is more common with larger breast implants (> 350 cc).

Sometimes the breast implant size, position and health is fine, but the breast tissue itself has sagged. In these cases a Breast Lift is often recommended to improve or restore breast aesthetics. Breast Lifts may be performed in conjunction with the Breast Implant Revision. In more complex cases, a delayed breast lift may be recommended. Case 1 - Bottoming Out (Inferior Malposition)

Breast Implant Revision Before and After Photos: The primary goal of Breast Implant Revision Surgery is to restore breast aesthetics. In this case, the breast implants have "bottomed-out." This is seen most frequently with breast implants larger than 350 cc. The larger (and heavier) the implants, the higher the incidence of "Bottoming Out." The above patient also has rippling of her breast implants, seen around the lower border of each breast.

Before: This patient was unhappy with the location of her breasts and wanted to raise them. She was happy with her size and did not want smaller implants, though this would decrease the risk of recurrence. She is thin with a small frame. Rippling is seen around the lower border of the breasts on both sides prior to her Breast Implant Revision.

After: The overall shape and location of the breasts is improved. The breast are significantly higher, and the nipples are in a good position. A capsulotomy was performed above the implants, which made space for them to move up. A capsulorrhaphy was performed along the bottom and sides to support the implants. Additional support was provided by the formation of a neopocket and a sling of capsule inferiorly. Additionally, the sling thickened the tissue covering the breast implant and reduced the rippling.

For this type of Breast Implant Revision, artificial mesh and ADM (acellular dermal matrix) is often used to reinforce the repairs. Downsizing the breast implants can help reduce recurrence, as smaller implants weigh less. Changing from Saline to a Cohesive Gel Silicone Breast Implants (Gummy Bear Breast Implants) can help reduce rippling. If the nipples were lower, or the IMF higher, a Breast Lift might be needed.

Case 2 - Symmastia (Uniboob, Bread Loafing)

Breast Implant Revision Before and After Photos: This case depicts Symmastia Correction, one of the most difficult Breast Implant Revisions. Her breast implants are too close together, and the skin between them is lifted. This situation is also known as "bread loafing" or the "uniboob" deformity, because in its worst case, the two breasts connect into one. Symmastia is associated with larger and high profile implants, tight skin and attenuated connections between the skin and the edge of the sternum. Reducing the projection of the breast implants is often recommended during Symmastia Repair, as this helps reduce the risk of recurrence.

Before: In this case, the breast implants are raising the skin off the chest over the sternum. The breast implants' pockets overlap in the midline, and her inframammary folds (IMFs) are connected. This is sometimes seen naturally in women with large breasts, but in this case the symmastia was created by her breast implants. When pushed toward the center one at a time, the breast implants crossed well past the midline.

After: The breast implants were reduced in profile. The overlapping capsules were closed medially and neopockets were created to define the cleavage. In some cases, additional reinforcement is needed in the form of an ADM (acellular dermal matrix) or a manufactured surgical scaffold. After Breast Implant Revision, a specialized surgical garment, known as a thong bra, is worn to support the repair externally. Symmastia is a difficult problem. Symmastia Repair is very important because once symmastia occurs, the problem often continues to worsen. Additionally, it is difficult to hide symmastia, even in clothes. While a supportive bra can help other problems with breast implant malposition, in the case of symmastia, it may just make it worse.

Case 3 - Postpartum Breast Sagging (Ptosis)

Breast Implant Revision Before and After Photos: Sometimes mother nature is the cause of Breast Implant Revision. Pregnancy and breast feeding frequently cause breast enlargement. After delivery and the cessation of lactation, the volume of the breasts decrease, leading to a deflated, saggy appearance termed ptosis. Ptosis happens with and without breast implants. Even with breast implants, the natural breast may still fall. The difference is that they fall off the implants. This is sometimes called the waterfall deformity, as is seen in the before pictures above.

Before: In this case, the breast implants were under the pectoralis muscles. As a result, the implants are supported and do not sag with the breasts. The breasts themselves, however, have sagged right off the breast implants, giving this elongated, waterfall deformity with the nipples well below the IMFs. Breast implants placed in front of the muscle would more likely fall with the breasts. But in either case, Breast Implant Revision for postpartum ptosis often requires a Breast Lift.

After: In this case, Breast Implant Revision included a Breast Lift to raise the breasts and reposition the nipples higher on the breast mound.

During Breast Revision Surgery is also a good time to change the breast implants. If the breasts have shrunk (atrophied) after pregnancy, larger breast implants can be inserted. If you have older generation saline or silicone breast implants, it may be advantageous to "upgrade" them at the same time. On the other hand, if your breast implants are up-to-date, a Breast Lift may be all you need to restore your pre-pregnancy breast aesthetics.

Case 4 - Breast Implant Deflation/Replacement

Breast Implant Revision Before and After Photos: Breast Implant Deflation is a common reason for Breast Implant Revision. In this case, the patient's left saline breast implant leaked and is completely deflated. Saline breast implants are "honest" implants. Deflation is clearly evident, and therefore no special testing is required. The saline inside the implant leaks out through even the smallest of holes. The body quickly and safely absorbs the saline, and the breast implant rapidly deflates, usually overnight. Replacement is not an emergency but should be done promptly to prevent the capsule from shrinking. Since the deflated implant needs to be replaced, it is a good time to consider changing the size or style of your breast implants. Most deflations are covered by a warranty. The implant is often replaced for free, and this credit can be used to upgrade your breast implants. Money may also be available to help pay for the replacement surgery. Be sure to check with your breast implants' manufacturer to see exactly what is covered.

Before: This patient was happy with her Breast Augmentation, until the left breast implant deflated. Her warranty covered replacing both the deflated left breast implant and the intact right breast implant. Since she was already having both replaced, she decided to go up a size. Some patients also decide to change from saline to silicone. The warranty still applies, but there is an up-charge for the more expensive silicone breast implants.

After: This Breast Implant Revision included Breast Implant Replacement and capsulotomies to expand the pocket to accommodate the larger breast implants. She was back to her usual activities in a few days. Compared to primary Breast Augmentation, the recovery period after breast implant replacement is usually much easier and faster.

Case 5 - Capsular Contracture

Breast Implant Revision Before and After Photos: This Breast Implant Revision patient had severe Capsular Contracture. There are four stages of capsular contracture, and this is an example of stage IV - obvious physical distortion associated with pain. Capsular contracture is one of the more common reasons for Breast Implant Revision. It usually requires surgical intervention with modification of the capsule and possible breast implant replacement.

Before: This patient complained of painful, hard breasts that have shrunk over the years. Capsular contracture often involves one breast, but in this case, both breasts appear angry, red and contracted. On physical examination they are hard and do not move. This is an example of Grade IV Capsular Contracture. There are four grades of capsular contracture:

  • Grade I - Soft: The breasts are soft and mobile. The capsule around the breast implants is not detectable. This is the most common result after Breast Augmentation.
  • Grade II - Firm: The breasts are a little firm to the touch, but they look good. The capsule around the breast implants is a little firm but no distortion is present. This is the second most common result after Breast Augmentation.
  • Grade III - Hard: The breasts are firm and less mobile. The capsule around the breast implants is tight, and the shape is beginning to change. This is the third most common result after Breast Augmentation.
  • Grade IV - Painful: The breasts are hard and immobile. The capsule around the breast implants is squeezing the breast implants, causing physical distortion. Fortunately, this is the least common result after Breast Augmentation.

After: This Breast Implant Revision required bilateral Complete Capsulectomies with breast implant replacement. The result is an expansion of the breasts with restoration of pain-free, soft, mobile breast mounds and loss of the tight, contracted, angry breast appearance. The breasts have been restored to a soft, tear-drop shape. They feel better to the patient and to touch. They fit better in clothing, and the patient is much happier with her aesthetically restored breasts.
