Reasons For Breast Breast Lift (Mastopexy)

The Breast Lift is a common procedure. It can be performed with or without a breast implant. When combined with breast augmentation, it is called a Mastopexy Augmentation, which both lifts and enlarges the breasts. It is estimated that around 40% of all patients with breast implants will have a Breast Lift during their lifetime. Sometimes, Breast Lifts are recommended at the time of the initial Breast Augmentation to improve breast aesthetics. Other times, lifts are performed after the breasts mature or sag due to pregnancy or fluctuations in weight.

Breast Lifts are often stand alone procedures. In cases when the breast volume is sufficient but the breasts sag, Mastopexy alone will provide correction. Mastopexy literally means breast suspension or fixing and is the medical term for a Breast Lift. It does not change the volume of the breast by itself, but mastopexy may be combined with Breast Augmentation or Breast Reduction to increase or decrease the size of the breasts, respectively.

There are many techniques of Breast Lifts, and the type of lift depends on the type of breasts being lifted. Bigger lifts are required for the correction of saggier breasts. Below you will see examples of the more commonly performed breast lifts. Additional details regarding the types of Breast Lifts can be found on our Breast Lift Page. Breast Lifts are always elective surgery. There are no medical indications for a breast lift that qualify for insurance coverage, unless it is part of a Breast Reduction. It is done almost exclusively for cosmetic purposes or as part of Breast Augmentation Revision Surgery.

These are of actual patients who have had their plastic surgery by Dr. Joseph Mele, MD, FACS, at our San Francisco Bay Area plastic surgery center. While our Breast Lift Before and After Photos were selected to demonstrate what is possible with this plastic surgery procedure, individual results will vary.

What To Look For In Breast Lift Before And After Photos

What can a Breast Lift do for you? It literally lifts the breasts and raises the nipple on the breast mound. The following are selected Mastopexy Before and After Photos that demonstrate actual results.

Below, different examples of Breast Lifts are narrated and illustrated with Breast Lift Before and After Pictures. It is important to know that different types of Breast Lifts are not interchangeable. The type of lift is selected to get the best results and minimize the scars; however, it cannot be chosen solely on the basis of the scar you would like. The best breast lift is determined by how much lifting is needed. In general, the less breast tissue that needs to be lifted, the shorter the scars and the longer lasting the results.

Case 1 - No Vertical Scar Breast Lift

Mastopexy Before and After Photos: The primary goal of Mastopexy is to improve breast shape and restore breast aesthetics. In this case, the breast are elongated and pendulous. This is seen most frequently with postpartum breasts or when large breasts shrink with weight loss. Longer breasts with low nipples and a narrow base are good candidates for the No Vertical Scar Breast Lift. This eliminates the vertical scar that runs on the front of the breasts from the areolae to the inframammary fold used in the usual inverted-T or anchor lift.

Before: This patient was unhappy with the length and laxity of her breasts. She wanted to raise the nipples back up to the front of the breast mound and reduce the size of her areolae. The common lift used in this situation has an anchor-shaped scar. A Vertical Breast Lift eliminates the horizontal scar located in the inframammary fold; however, a vertical lift would be difficult due to the low set and vertically elongated anatomy of her breasts. On the other hand, her breasts are shaped well for a No Vertical Scar Breast Lift.

After: The above patient was happy with her size of her breasts before surgery, so a No Vertical Scar Breast Lift was performed. She has relatively large breasts, so a Breast Reduction would have increased the longevity of her results by decreasing the weight of the breasts, but it comes at the cost of smaller breast mounds. After her Breast Lift, her breasts are significantly higher; the areolae size is reduced, and the nipples are in a more aesthetic position. The overall shape, tone and elevation of her breasts is improved.

Case 2 - Inverted-T Breast Lift (Anchor Scar)

Breast Lift Before and After Photos: This case depicts a Breast Lift for grade III ptosis, i.e. sagging with the nipples pointing down. Her breast size is proportionate; however, the majority of the tissue in the breast mounds is located below the inframammary folds, and the nipples are way too low.

Before: In this case, the patient is happy with the amount of breast volume. In fact, in a bra, she is satisfied with the size and shape of her breast mounds; however, without a bra, the breast tissue and the nipples sit way too low.

After: The breasts and nipples are elevated. An anchor incision was used to elevate the nipples and raise the breast tissue. The scars are located around the areolae, down the breasts from the areolae to the inframammary fold (IMF) and in the IMF itself. The lateral half of the incision is hidden in the fold under the breast in the picture above. A front view of this type of lift can be seen below in Case 5

Case 3 - Pancake Breasts

Mastopexy Before and After Photos: Sometimes the breasts are both low and flat. In the case above, a breast lift was used to elevate and form the breasts into a more youthful shape.

Before: The breasts have a low, deflated and pancaked appearance. As a result, they are loose and hard to control, even with a bra. A Breast Lift was needed to elevate and reshape the breasts.

After: An Inverted-T Mastopexy was used to reduce the sagging breast tissue that used to be below the IMF. The shape of skin removal in the above Breast Lift not only raised the breasts but it also gave the breasts more projection and firmness. The nipples are positioned higher and are more prominent after her procedure, and the areolae are reduced in diameter.

Case 4 - Breast Augmentation Revision

Periareolar Breast Lift Before and After Photos: Smaller elevations in nipple position and areolar reduction can be achieved with the Periareolar Mastopexy. This type of breast lift uses a scar around the areolae, which can be very effective for carefully selected patients. On the other hand, if a larger lift or areolar reduction is needed, the periareolar lift is not as effective as the Vertical or Inverted-T type breast lift, and trying to lift too much may result in a stretched out and flattened nipple areolar complex.

Before: This patient has had Breast Augmentation. She was unhappy with the prominence of her previous periareolar scar and wanted larger breast implants. The scar is more noticeable because it was placed within the pigmented skin around the nipple instead of at the junction of the areola and the surrounding nonpigmented skin.

A Periareolar Mastopexy was performed in conjunction with a Breast Implant Exchange and areolar reduction as parts of her Breast Augmentation Revision.

After: This patient was much happier after her Breast Implant Replacement and Periareolar breast lift. The breast size was more to her liking. The nipples and areolae were reduced in diameter, and the nipples were raised.

Case 5 - Asymmetrical Mastopexy

Mastopexy Before and After Photos: This patient had breast asymmetry and sagging prior to her Inverted-T Scar Breast Lift. If the breasts are different shapes but have the same volume, a breast lift will correct the asymmetry. If one breast is larger than the other, breast tissue is removed from the larger breast to even out the results. In cases were more upper pole fullness is desired, a low-profile breast implant is placed in the upper breast.

Before: This patient complained of asymmetrical and deflated breasts. An anchor scar was used to elevate the nipples, reduce the areolae and reduce the depth of the inframammary fold (IMF).

After: The scar runs around the areolae, from the bottom of the areolae to the middle of the IMF and in the IMF, loosely making the shape of an anchor. Overall, her symmetry is improved. Her breasts are much firmer and in a more aesthetic position on the chest wall.

Case 6 - Breast Implant Revision for Waterfall Deformity

Breast Lift With Breast Lift Before and After Photos

Breast Lift Before and After Photos: Pregnancy is a common reason for Breast Lifts. The above patient had Breast Augmentation prior to having children. Her submuscular breast implants have remained in the correct position; however, with lactation, the breast tissue expanded. After breast feeding ceased, the breasts shrunk and dropped (involuted). This appearance of deflated breast tissue draping off the breast implants is called the "Waterfall Deformity", and a Breast Lift is just what the doctor ordered to cure the deformity.

Before: This patient was happy with her Breast Augmentation, until her breasts changed after pregnancy. The resulting waterfall deformity was corrected with a breast lift, leaving the previously placed breast implants in their subpectoral pockets.

After: This Breast Implant Revision consisted of only a Breast Lift. The breast implant was in a good position, so the breast tissue was lifted up over the implant to correct the shape and restore her pre-baby breasts. Since the breast implant did not need to be modified, her recovery was faster and postoperative pain was minimal.
