Tuberous Breast Augmentation Before & After Pictures

Tubular Breast Deformity is the most common of all congenital breast deformities. It is characterized by small, abnormally shaped breasts. Characteristically, the breasts are asymmetric with a constricted base, hypoplastic (little) breast tissue, a herniated nipple-areola complex, deficient skin and an elevated inframammary fold (IMF). Some studies have the incidence as high as 80% in women with breast asymmetry. Patients with tubular breasts often seek Breast Augmentation. Tubular breast deformity may involve one or both breasts. When bilateral, one side is almost always more affected than the other. In fact, asymmetrical breast development may be the first sign of tuberous breasts.

Correction of Tuberous Breasts is quite varied, because the range of deformity is large. Affected breasts may look very close to normal or quite bizarre, and it is not unusual to have only one breast involved.

How To Correct Tuberous Breasts

Tuberous breasts are also called Tubular Breasts, constricted breasts, lower pole hypoplasia, Snoopy nose deformity, narrow based breasts and herniated nipple areola complex. The most common procedures used to correct Tuberous Breasts are Breast Augmentation and Breast Lift. Frequently, the combination of the two, Mastopexy Augmentation, is required to get the best aesthetic results. Staged surgeries may also be necessary, especially in younger women whose breasts are still developing.

The combination of Mastopexy Augmentation is the most common corrective surgery used for the correction of tubular breasts. Breast Implants certainly help when the breasts are small. The use of different sized breast implants can also correct mild breast asymmetry; however, reducing the larger breast is a better option in cases with significant differences in breast tissue development.

A Breast Lift is useful for reshaping the breasts, both individually and for symmetry. Problems like low set nipples and puffy nipples are not improved with breast augmentation and may potentially worsen. Since these are two common manifestations of tubular breasts, a breast lift is often required to correct the Snoopy nose deformity. In tuberous breasts, the nipples appear to be low on the breasts because the lower half of the breasts do not develop fully. By raising the nipples and rounding the breasts with a lift, the breasts achieve a perkier and more feminine shape.

Breast Implants can be silicone or saline, smooth or textured, round or shaped, and they can be placed in front of or behind the pectoralis major muscle. Breast Lifts can be periareolar, vertical or anchor shaped. There are advantages and disadvantages to each option. Dr. Mele will review your choices, the risks and benefits, and help you decide which is most likely to get you the results you desire.

While a lot of effort has been made to provide the latest and most accurate information about the options for Tuberous Breast Augmentation, the fine details are best discussed during an in-person appointment with a Board Certified Plastic Surgeon like Dr. Joseph A. Mele, MD, FACS. Call his San Francisco Bay Area Plastic Surgery office today at (925) 943-6353, to schedule a private consultation.

Case 1 - Type II Both Lower Quadrants Deficient -

Tubular Breast Augmentation Before and After Photos

Tubular Breast Augmentation Before and After Photos: The hallmark of Tuberous breasts is underdevelopment (hypoplasia). Hypoplasia of the lower inner (medial) quadrant of the breast (Type I) is the most common, while hypoplasia of all four quadrants (Type III) is the least common. The case above is Type II, with hypoplasia of the lower half of the breast.

Before: The nipples are low, the inframammary folds (IMFs) are high and the base of the breast is constricted on the right more than the left. Underdevelopment of the lower half of the breast is visible with deficient breast volume and skin below the areolae. The upper breast and nipple areola complexes have developed normally without breast tissue herniation. Lastly, the patient was unhappy with the loss of breast volume after multiple pregnancies.

After: A Breast Augmentation was performed to restore volume in the lower pole of the breasts. The nipples appear to have been elevated, but actually the IMFs were lowered to an aesthetic position, and the breast volume was restored with the implant. Her breasts now have a natural and youthful appearance. I can't sum up her results any better than she did:

I am more confident and feel romantic. For me it's tank tops on casual days and well fitting dresses on formal nights which would not otherwise not occur if I had not undergone breast augmentation. Breast augmentation has totally changed my life.”

Case 2 - Type III All Four Quadrants Deficient

Tuberous Breast Augmentation Before and After Photos

Tubular Breast Augmentation with Vertical Lift Before and After Photos: Type III Tuberous breasts have hypoplasia of the entire breast. The only area that has developed breast tissue is directly beneath the nipples, and it is herniating into the areolae. She has such severe hyperplasia of the lower half of her breast that the areolae border the IMF.

Before: The average breast has two inches of skin between the lower edge of the areola and the IMF; she has none. The small amount of breast tissue she has is herniating into the areolae, leaving her with a masculine chest.

After: To create feminine breasts, I performed a Mastopexy Augmentation, specifically Breast Augmentation with a Vertical Lift. This enhanced her breast mound shape and volume and elevated her nipples to the front of her breasts. A small areolar reduction was also performed to help alleviate herniation of the breast tissue. You can tell by her tan that she is much happier with her new curves.

Case 3 - Type II Both Lower Quadrants Are Deficient

Tubular Breast Augmentation Before and After Photos

Tubular Breast Augmentation with Periareolar Lift Before and After Photos: This patient has Type II bordering on Type III tuberous breast deformity. She has low set nipples due to severe lower pole hypoplasia and the typical shape of Tuberous Breasts.

Before: In this case, the lower half of the breasts have not developed. Breast hypoplasia is combined with lower pole constriction and a paucity of skin. She has some upper pole breast tissue and areolar herniation of the central breast tissue.

After: Breast Augmentation with a Periareolar Lift was planned to enlarge and shape her breasts. The lift allows for elevation of the nipples, reduction of the areolar breast tissue herniation and creates more skin below the nipples for the breast implant to fill. The areolae enlarge and are restricted with the amount of elevation possible with this technique. A vertical lift can raise the nipples more and control the size of the areolae better, but it comes at the price of an additional scar on the front of the breast which runs between the areola and the IMF.

Case 4 - Type I Lower Inner Quadrant Deficiency

Tuberous Breast Augmentation Before and After Photos

Tuberous Breast Augmentation with Periareolar Lift Before and After Photos: Above is an example of Type I Tubular Breasts. Her breast hypoplasia is mainly in the lower inner quadrants. She has breast tissue in the upper and lateral quadrants, but the constricted base gives an elongated, saggy appearance. Unlike example 3 above, she has a small amount of skin between her nipple and IMF.

Before: This patient desired larger breasts and nipple elevation. She has normal cleavage of the upper breasts, but is missing breast tissue in the lower, inner aspect of the breast. She also has mild skin excess with respect to the amount of breast tissue present. Breast Augmentation alone (without a lift) would push her nipples even lower on the breasts.

After: Breast Augmentation with a Periareolar Lift was performed, giving her larger and perkier breasts. After surgery, her nipples are located on the front of her breasts, and now her breasts are teardrop shaped.

Another name for Tuberous Breasts is Snoopy Nose Deformity. It is easy to imagine her left breast in the preop picture as Snoopy's nose. The elongation is corrected with the combination of a breast lift and augmentation.

Case 5 - Type II Both Lower Quadrants Are Deficient

Tubular Breast Augmentation Before and After Photos

Tubular Breast Augmentation with Periareolar Lift Before and After Photos: This patient has puffy nipples caused by breast tissue herniating into the nipple areola complex. She lacks lower breast development and has minimal upper breast tissue. She desired breast enlargement and correction of her nipples.

Before: There is no skin available to augment below her puffy nipples. The IMF is also high. The plan was Mastopexy Augmentation to raise the nipples and provide volume for her breast mounds.

After: The skin needed for Breast Augmentation below the nipple was created by elevating the nipple with a periareolar lift. The lift also helps flatten her puffy nipples. A breast implant was used to provide volume and increase projection of the breasts.

Case 6 - Type III All Four Quadrants Deficient in a Man

Tuberous Breast Augmentation Before and After Photos

Tuberous Breast Reduction Before and After Photos: Men can also have tubular breasts. For men, the lack of lower pole development and low, laterally placed nipples is not a problem. In fact, this is normal masculine anatomy. The problem is the large, puffy nipples. Even in a shirt, the fullness is obvious and embarrassing. Removing the breast tissue is often enough; however, the more tubular the deformity, the more likely a periareolar reduction may be needed.

Before: This patient has a normal chest; however, he has puffy nipples due to gynecomastia and tuberous breast deformity. In this case, the areolae diameters are reasonable and are expected to shrink with removal of the breast tissue.

After: Postoperatively, his areolae are flatter and smaller. He can wear a shirt without embarrassment, and can remove his shirt comfortably, revealing his masculine chest. The resection was performed using an incision placed around the lower edge of the areolae that tends to blend into the normal color change, making it difficult to see.
